  • Parkinson's disease

  • Post-traumatic stress disorder (PTSD)

  • Severe chronic or intractable pain

  • Sickle cell anemia

  • Terminal illness

  • Tourette syndrome

How Do I Apply for a Medical Marijuana Card?

Step 1: Register with the State

After confirming your condition qualifies, create a profile on the PA Department of Health registry. Use your PA state-issued ID or Driver's License exactly as it appears (all caps).

Step 2: Schedule a Doctor's Appointment

Book a telemedicine appointment with a state-approved practitioner such as Green Bridge Society to receive your certification.

Step 3: Pay the Annual Fee

Log in to your PA Medical Marijuana Program dashboard and pay the $50 annual fee after your appointment. Discounts are available for Medicaid and SNAP recipients. Your card arrives in approximately 21 days, mailed to your profile address.

Are There Any Age Limitations?

Applicants must be 18 years or older. Minors may also qualify but require a parent or legal guardian to act as a caregiver.
