  • Positive status for HIV/AIDS

  • Post-Traumatic Stress Disorder (PTSD)

  • Seizure disorder, including epilepsy

  • Terminal illness with prognosis of less than 12 months to live

  • Tourette Syndrome

How Do I Apply for a Medical Marijuana Card?

Step 1: Get Provider Certification

After confirming your condition is on the approved qualifying list, use the MCP public registry to find a participating healthcare practitioner. Your provider will issue an Authorizing Provider Statement with a reference ID and registry ID needed to create your account.

Step 2: Register with the State

Create an account on the NJ MCP patient portal using your reference ID and registry ID. Complete registration by identifying an Alternative Treatment Center, uploading proof of NJ residency, and uploading a recent front-facing photograph.

Step 3: Receive Your ID Card

Choose a free digital ID card or a physical card for a $10 fee. Your Medical Cannabis Program card is valid for two years; renewal reminders are sent ~60 days before expiration.

Applicants must be 18 years or older. Minors may also qualify but require a parent or legal guardian to act as a caregiver.
